Carl Forthman, 50, went to his heavenly home on July 3, 2019.

Carl is survived by his father and step mom Joe and Jennett Forthman; his mother Kathy Lewis; a sister, Angie Deiss; two brothers, Keith Ekensiar and Jason Forthman; and his fianceé, Brandon Sheldon. He is also survived by four daughters, Lacey Jo Hensley of Marvell, Arkansas, Tasha Rae Henry of Oklahoma, McKenzie Rose Forthman of Indiana, and Alexandria of Indiana; and three grandchildren and one on the way.

Services will be held 1 p.m. Friday, July 5, 2019, at Freedom Church, 1361 Christian Ave., Noblesville, IN. Visitation will be from 11 a.m. until service time at the church.

Local services will be held at 11 a.m. Saturday, July 6, 2019, at Oak Grove Baptist Church in Mantachie with Bro. Rodney Lindsey officiating. Visitation will be from 9 a.m. until service time at the church.

Carl will be laid to rest at Oak Grove Cemetery in the Ratliff community of Itawamba County, Mississippi.
